My child still does not have consent even though we submitted a form

We suggest that you send in another form by going here and that you confirm that the username and all information listed on it corresponds with all the information in your account.

Also, please take care to ensure that the signature is clear and legible. We can only accept the signature of a direct parent or legal guardian of the child, and for a legal guardian it must be clearly denoted on the form that this is the case. We unfortunately cannot accept signatures from relatives of the child unless they are the lawful guardian of the child.
